Requirements Traceability Matrix is a document, usually in the form of a table, that correlates any two baselined documents that require a many-to-many relationship to determine the completeness of the relationship.

It contains such information as Revision History and Distribution List.

The requirements traceability matrix contains the following fields: Unique identification number containing the general category of the requirement, Requirement statement, Requirement source (e.g. Configuration Control Board), Software Requirements Specification/Functional Requirements number containing the requirement, Module containing the requirement, Test Case number(s), Verification of successful testing of requirements, Modification field, Comments.
